Consent of the subject of personal data

By marking the consent to personal data processing, I confirm that I am familiar with the Law of Ukraine "On Personal Data Protection" and the purpose of personal data processing*, and I provide permission for processing of my personal data in accordance with the Law of Ukraine "On Personal Data Protection".

*Purpose of personal data processing: Processing and execution of the personal data subject's request/order, fulfillment of the contractual obligations to the personal data subject, ensuring implementation of the tax relations, performance of the functions, powers and duties imposed on the personal data owner in accordance with the current legislation of Ukraine, performance of other duties imposed by the law to the personal data owner. Personal data is processed and retained as long as necessary to achieve the purposes for which it was collected, including to meet any legal, accounting or reporting requirements, or until the personal data is deleted by the personal data subject or at the request of the personal data subject.
